# Alpha memo: metformin resistance cross-context evidence signal
**One-sentence alpha:** Preclinical rat work suggested metformin plus swimming training may additively improve insulin sensitivity, whereas a human DARE re-analysis suggests metformin use may attenuate exercise-driven HbA1c and fitness gains — the same anchor splits by context.
**Receipt 1:** Effects Of Metformin Administration With Swimming TVaining In Fructose Induced Insulin Resistance Rats (2007) — in fructose-fed insulin-resistant Wistar rats, the authors evaluate whether adding metformin to 45 min/day swimming training (5 days/week) increases insulin sensitivity over either alone, framing it as an open combinatorial question.
**Receipt 2:** Does metformin modify the effect on glycaemic control of aerobic exercise, resistance exercise or both? (2013, DARE re-analysis) — in adults with type 2 diabetes randomised to aerobic, resistance, combined, or waiting-list control, metformin users versus non-users may attenuate glycaemic and fitness responses to the exercise arms, and aerobic training was associated with a significant HbA1c reduction in metformin users versus control.
**Why this is surprising:** Receipt 1 made plausible that metformin transports as an additive insulin-sensitiser on top of exercise, but Receipt 2 updates that possibility toward interference or attenuation at the clinical translation boundary.

**Caveats/falsifiers:**
- Receipt 1 is a small-sample (N=8/group) fructose-fed insulin-resistant male Wistar rat model; Receipt 2 is a 251-adult, metformin-using subgroup from a 22-week DARE re-analysis, so species, dose, route, duration, baseline status, and sample size all differ and the metformin-as-moderator hypothesis is tentative and confounded by the other axes — treat as a heterogeneous cross-context signal rather than a direct overturning, and no clinical, dosing, or supplementation recommendation follows.
- A decisive falsifier would be a within-trial factorial in humans with type 2 diabetes that randomises metformin vs. placebo within an aerobic, resistance, and combined exercise arm and reports non-attenuated HbA1c and VO2max gains in metformin users, or a rat replication in a non-fructose, non-insulin-resistant model showing the additive effect holds across sexes.
