# Alpha memo: Resveratrol–exercise combination in older adults may not improve TMAO or cardiovascular markers despite liver anti-aging signals in SAMP8 mice
**One-sentence alpha:** Mechanistic findings in mature SAMP8 mouse liver suggesting combined resveratrol and exercise blunt age-related fibrosis and apoptosis did not translate to detectable TMAO or CVD-marker benefits over 12 weeks in older adults at two dosages.

**Receipt 1:** Combination resveratrol with exercise training has anti-aging function only at mature stage SAMP8 mice liver — in 6-month-old SAMP8 mice versus 3-month-old, combined habitual exercise training and resveratrol intake reduced hepatic adipocytes, collagen deposition, and apoptosis, and modulated PI3K/Akt/Bad/cytochrome c/Bcl-2 and ERK5/STAT3/IL-6 pathways.
**Receipt 2:** Effects of resveratrol on changes in trimethylamine-N-oxide and circulating cardiovascular factors following exercise training among older adults — in 41 older adults (mean age ~72 y) with high CVD risk undergoing a 12-week, 2×/week supervised multi-component exercise program randomized to 500 mg/day, 1000 mg/day resveratrol, or placebo, the combined intervention failed to produce the expected reductions in circulating TMAO and related CVD-linked metabolites.

**Why this is surprising:** Receipt 1 made plausible a clean positive translation of the resveratrol-plus-exercise signal across tissues, while Receipt 2 updates that this same anchor combination splits or fails at the human systemic-TMAO/Cardiovascular readout, showing the benefit does not generalise.

**Caveats/falsifiers:**
- Receipt 1 is in SAMP8 mice at one mature time-point with liver histology and protein markers, not functional CVD outcomes; Receipt 2 is a 12-week, two-dose trial in older adults with high CVD risk measuring circulating TMAO and related metabolites, not hepatic aging.
- A decisive falsifier would be a longer-duration (≥6 months) human trial with multiple resveratrol doses and combined aerobic+resistance training showing dose-dependent reductions in TMAO and a hard CVD endpoint (e.g., arterial stiffness or event reduction) that closes the gap with the SAMP8 mechanistic picture.
