# Alpha memo: metformin exercise protocol mismatch
**One-sentence alpha:** Receipt 1 made plausible an additive or enhancing signal between metformin and exercise training on fasting insulin secretion in prediabetes, while Receipt 2 updates this by suggesting the metformin–exercise interaction may attenuate glycaemic response in type 2 diabetes depending on exercise modality.
**Receipt 1:** Metformin Enhances the Effects of Exercise Training on Fasting Insulin Secretion in Adults with Prediabetes (2010, n=32) — a double-blind RCT in overweight sedentary adults with prediabetes randomized to placebo, metformin, exercise+placebo, or exercise+metformin, designed to assess changes in fasting insulin secretion and insulin resistance.
**Receipt 2:** Does metformin modify the effect on glycaemic control of aerobic exercise, resistance exercise or both? (2013, DARE trial) — in 251 adults with type 2 diabetes (143 metformin users, 82 non-users) randomized to aerobic, resistance, combined training, or control for 22 weeks, aerobic training reduced HbA1c in metformin users versus control, with prior literature cited as suggesting metformin might attenuate exercise effects on glycaemia or fitness.
